| Insurance We will be happy to file insurance for you as a courtesy. We will need a copy of your insurance card. You must also realize that your insurance is a contract between you, your employer, and the insurance company. We are not a party to that contract. All charges are your responsibility from the date the services are rendered. Also note that not all services are a covered benefit in all insurance contracts. If you have insurance, we require you to pay your portion of the charges as a down payment at the time of service. You will be asked to pay for adult fluoride treatments and sealant treatments at the time of visit, unless your insurance covers these procedures.

If you have dental insurance, we will work with you and your insurance company. However, it is important to know that if your insurance company does not pay the estimated amount within 45 days, we will expect you to promptly make the payment.
Please contact your employer to obtain precise information regarding your plan’s benefits, deductions, and limitations. Kindly, present your insurance card to our receptionist at the time of your visit. We, unfortunately, do not participate in any Health Maintenance Organizations (HMOs) or Dental Maintenance Organizations (DMO's).
